Linn County Public Health, in partnership with area agencies through the My Care Community initiative, is excited to announce a new website functionality to help people get connected to needed resources and programs.
People in need of assistance can now submit a request through a simple and secure web form. The form will be submitted to a local care coordinator, who will assess their needs and electronically refer them to resources. By using technology, My Care Community simplifies the traditional referral process by reducing much of the paperwork, phone calls, and repeat appointments, which lifts the burden off the individual who is seeking services. People can also get care by visiting any My Care Community partner organization and requesting help.
“This new enhancement to the My Care Community initiative is able to streamline local organizations in assisting individuals in our community,” said Dr. Pramod Dwivedi, Linn County Public Health Director. “We are excited to take this initiative to a new level and offer an easier and more convenient way for individuals to get the care they need most. This work would not be possible without over 70 organizations in East Central Iowa.”
My Care Community, Partners for Better Care© is a cross-sector collaborative that has served East Central Iowa since 2017. This initiative aims to improve population health outcomes and make it easier for individuals and professionals to find and access services, ranging from medical care, access to food, housing or utility assistance, transportation, and more. My Care Community is open to anyone who lives in, works in, or visits East Central Iowa.
